Introduction We are very fortunate to have restaurants in walking distance from the studio (ex. Subway, Scoop 33). Dancers taking multiple classes per night may wish to visit said restaurants during a “dance class break” which may occur before, after, or in between dance classes. However, it has come to our attention that some dancers have been behaving inappropriately as we have received several complaints. Therefore, dancers wishing to leave the studio unsupervised, must have a signed permission slip/liability waiver on file at Dance Innovations Dance Center LLC. Dancer Responsibilities and Permission I agree to be courteous and respectful to the employees, patrons, and property while getting food at an outside establishment during my dance class break. I will be conscious of the time and will not be late to my next class. I will keep the lobby clear of any food remnants. If an issue arises, I realize that my parent/guardian will be notified. I give permission for my dancer to leave the premises of Dance Innovations Dance Center LLC without parent or guardian supervision. I realize I may be contacted by DIDC or by an outside establishment if an issue arises while my dancer is off of the studio premises during a dance class break.
